Use this form to fax an order for parts and accessories. Print this 2-page form, then fax it to 805.466.4782 and retain the printed copy for your records. You will receive two emails from us associated with your order. The first will be an order confirmation; the second email will contain shipping information and the exact amount your credit card has been charged. Shipping charges for your order cannot be calculated until the order is packed and ready for shipping. We ship via UPS Ground, and you are charged only the exact UPS rate for shipping from our facility to you, plus a handling charge of $5.95 on each order to cover the cost of professional packing and packing materials.
